🐶 Introduction
Diphenhydramine (Benadryl®) is a first-generation H1 antihistamine and anticholinergic agent commonly used in veterinary practice for allergic reactions, mild urticaria (hives), vaccine reactions, motion sickness, and mild anxiety. It is one of the few OTC antihistamines considered generally safe for dogs when dosed correctly.
Unlike second-generation antihistamines (cetirizine, loratadine), diphenhydramine crosses the blood-brain barrier, producing sedation — a side effect that can be both useful (motion sickness) and a monitoring concern. Its onset is approximately 30 minutes with a duration of 4–8 hours.
📚 Clinical Pharmacology & Dosing Guidelines
Standard Dose (All indications)
- 1 mg/kg PO every 8 hours (q8h) — standard clinical dose
- Maximum single dose: 2 mg/kg or 50 mg (whichever is lower)
- For anaphylaxis / severe reactions: 2–4 mg/kg IV/IM (clinic use only — a veterinarian must administer)

Contraindications
- Glaucoma (anticholinergic effect raises intraocular pressure)
- Benign prostatic hyperplasia or urinary retention
- Cardiac arrhythmias, hypertension
- Hyperthyroidism, seizure disorders
- Concurrent use of MAO inhibitors or other CNS depressants
