🐶 Introduction
Tramadol is a centrally acting synthetic opioid analgesic that works via two complementary mechanisms: weak μ-opioid receptor agonism and inhibition of serotonin and norepinephrine reuptake. In humans, its analgesic effect relies primarily on conversion to the active metabolite O-desmethyltramadol (M1) via the CYP2D6 enzyme.
Important canine-specific pharmacology: Dogs are “poor converters” of tramadol to M1 — they produce significantly less of this active opioid metabolite compared to humans and cats. This means the analgesic efficacy of tramadol in dogs is more dependent on its monoamine reuptake inhibition than opioid activity. Clinical evidence for tramadol’s analgesic effectiveness in dogs is mixed, but it remains widely used, particularly for chronic musculoskeletal pain and as an adjunct analgesic.
📚 Dosing Guidelines
Acute Pain / Post-operative
2–5 mg/kg PO q8–12h. For immediate post-operative use; typically combined with an NSAID (e.g., meloxicam) for multimodal analgesia.
Chronic Pain (Osteoarthritis, Cancer Pain)
2–5 mg/kg PO q8–12h. Begin at the lower end; titrate based on response. Monitor for sedation, especially in older dogs.
Severe / Refractory Pain
Up to 10 mg/kg PO q8h (with caution, under veterinary supervision). Higher doses significantly increase risk of serotonin syndrome.

- 50 mg tablets (most common human formulation)
- 100 mg tablets (extended-release — NOT recommended for dogs; use immediate-release only)
- Compounded: 10 mg/mL or 20 mg/mL oral liquid
Contraindications & Drug Interactions
- ⛔ MAO inhibitors (e.g., selegiline/Anipryl) — serotonin syndrome risk
- ⛔ SSRIs / TCAs — additive serotonin effects
- ⚠️ Other CNS depressants (opioids, benzodiazepines, phenobarbital) — additive sedation
- ⚠️ Epileptic dogs — tramadol lowers seizure threshold
